std::size_t madspace::select_combine_channel_index(const std::vector<std::size_t> &cum_counts, std::size_t random_index)

select_combine_channel() over a plain cumulative-count vector, for testing.

Returns cum_counts.size() if random_index is out of range, i.e. not below the total.
